I authorize Coaxial Cable TV Corp to charge my credit/check card each month until I notify Coaxial Cable to disconnect my service.

By signing this form, I understand that:

  1. The payment will be charged to my credit/check card on the first business day of each month for the current month's service and any pay-per-view purchases charged to my account.
  2. If my credit/check card is declined for any reason, my service will be disconnected immediately.  If I choose to reconnect my service, I agree to pay any applicable reconnect fees and any balance due at the time of reinstallation.
  3. No more than one credit card will be billed for each account.
  4. If I wish to cancel this agreement, I must contact Coaxial Cable during regular business hours at 814-734-1424 and will return all equipment to Coaxial Cable at that time.
  5. If my credit card or bank account changes for any reason, I will also notify Coaxial Cable. I will also notify Coaxial Cable of a change in the credit card expiration date. If I fail to provide this information prior to the due date and Coaxial Cable is unable to process my payment, I will be responsible for an alternate payment arrangement. If I do not contact Coaxial Cable or make other arrangements, I may be subject to disconnection.
  6. This agreement will remain in effect until canceled by either party. If, canceled, I acknowledge that I will be billed for any outstanding amounts due and agree to pay such amounts in full.
  7. On the first business day of the month following my disconnect (at the end of the school term/school year/my request), my card will be charged for any outstanding pay-per-view purchases, etc. accumulated on my account following my last payment.

Your account will be charged as you requested, in the amount of your services for the current month plus and pay-per-view purchases that have been charged to your account.
